Thank you for booking a tasting consultation with us. The next step is to complete this form and submit to us your flavor selections for the appointment. Please note that until we receive your flavor selections we consider your appointment tentative as we do not conduct appointments without this information. The form must be completed at least 48 hours prior to the scheduled appointment. 

**Cake tasting appointments please select up to 6 cake flavors and 14 fillings and buttercreams
**Dessert display appointments please select up to 4 cake flavors and 8-10 fillings and buttercreams in addition to 6-8 dessert options we will be providing.
